In order to be eligible to request equipment, you must read and understand the following EQUIPMENT AGREEMENT:

I agree to return all of the items listed on the request form by the set time.

I understand that any equipment returned after that time is subject to a fine.

I agree that I am solely responsible for any damage to, loss of, or misuse of this equipment while it is in my possession.

I verify that I know how to operate this equipment. Instruction on the proper use of equipment is available from the ATS staff.
